Сердечно-сосудистые заболевания остаются высокоактуальной проблемой современного здравоохранения, ассоциированной со значительными экономическими издержками и потерей трудоспособности населения. Серьезную угрозу представляют аневризмы грудной аорты: они часто развиваются и протекают бессимптомно. Диагностика аневризм нередко происходит на поздних стадиях, что приводит к высокой летальности при развитии связанных с ними осложнений. Поэтому необходимо проведение новых эпидемиологических исследований по данной проблеме в нашей стране.</p></sec><sec><title>Цель</title><p>Цель: изучить распространенность патологического расширения грудной аорты среди населения г. М осквы по данным компьютерной томографии (КТ) органов грудной клетки (ОГК) с помощью технологий искусственного интеллекта (ТИИ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведен ретроспективный анализ данных КТ ОГК, выполненной по общим показаниям у 227 149 пациентов за период с октября 2022 г. по октябрь 2023 г. с применением ТИИ.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлено, что 13,3% пациентов имели признаки дилатации грудной аорты, а у 0,8% обнаружены аневризмы. Распространенность аневризм грудной аорты в г. Москве составила 12,4 случая на 100 тыс. человек. Мужчины более подвержены данной патологии, чем женщины, среди них преобладали аневризмы нисходящей аорты, а среди женщин – восходящей. Частота встречаемости патологии грудной аорты увеличивается с возрастом. Обсуждаются возможные причины и пути минимизации ошибок в работе ТИИ.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты исследования подчеркивают важность оппортунистического скрининга аневризмы грудной аорты с применением ТИИ, особенно у мужчин и пожилых пациентов, с целью своевременного выявления и предотвращения осложнений этого серьезного заболевания. Подтверждена эффективность использования ТИИ в качестве системы поддержки принятия врачебных решений для врача-рентгенолога при анализе результатов КТ ОГК. Настоящая работа вносит важный вклад в обновление данных по распространенности аневризм грудной аорты у асимптомных пациентов в России. Полученная информация позволяет более эффективно направлять усилия по диагностике и профилактике рассматриваемого заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Cardiovascular diseases remain a pressing issue associated with great economic burden and loss of earning capacity. Among them, thoracic aortic aneurysms pose a serious threat as they commonly develop asymptomatically. Patients with aneurysms are often diagnosed late, which contributes to higher mortality rates due to complications. For this reason, conducting new epidemiological studies on this problem in our country is relevant.</p></sec><sec><title>Objective</title><p>Objective: to study the prevalence of pathological dilatation of the thoracic aorta in Moscow by means of artificial intelligence technologies (AITs) using chest computed tomography (CT) data.</p></sec><sec><title>Material and methods</title><p>Material and methods. A retrospective analysis of chest CT data from 227,149 patients obtained in the period from October 2022 to October 2023 was performed using AITs.</p></sec><sec><title>Results</title><p>Results. The analysis revealed that 13.3% of patients exhibited signs of thoracic aortic dilatation, while 0.8% had aneurysm signs. The prevalence of thoracic aortic aneurysms in Moscow was 12.4 cases per 100,000 individuals. Males were more susceptible than females; aneurysms of the descending aorta were more typical for them, while aneurysms of the ascending aorta were more common in females. The incidence of thoracic aortic dilatation increased with age. Potential causes and strategies to minimize AIT errors were discussed.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results highlight the importance of opportunistic screening for thoracic aortic aneurysms to ensure timely detection and prevention of complications. It would be especially beneficial to men and elderly population. The effectiveness of AITs to support decision-making by radiologists analyzing chest CT results was confirmed. The study provides an important update to the data on the prevalence of thoracic aortic aneurysms in asymptomatic patients in Russia.
