В исследование включена 271 женщина, прошедшая магнитно-резонансную томографию (МРТ) органов малого таза в период с августа по ноябрь 2021 г. Два рентгенолога с опытом работы 6 лет (исследователь 1) и 2 года (исследователь 2) ретроспективно провели анализ МРТ-изображений, при котором все образования придатков матки оценивались по категориальной шкале O-RADS MRI от 0 до 5 баллов. Осуществлен анализ истинно-положительных, истинно-отрицательных, ложноположительных и ложноотрицательных показателей в соответствии с данными МРТ по сравнению с референсными данными (результатами гистологического исследования или клинико-инструментального наблюдения через 1 год).</p></sec><sec><title>Результаты</title><p>Результаты. Установлена высокая диагностическая информативность категориальной шкалы риска злокачественности образований яичников O-RADS MRI (чувствительность 87,5% и 87,5%, специфичность 97,84% и 96,75%, площадь под кривой (area under curve, AUC) 0,991 и 0,986 для исследователей 1 и 2 соответственно), а также хорошая согласованность между исследователями (коэффициент каппа Коэна 0,83). Проанализированы ошибки исследователей, а также спорные вопросы категориальной принадлежности образований, вызвавшие ложноположительные и ложноотрицательные результаты. Составлены наглядные пособия по МРТ-визуализации образований яичников в соответствии с классификацией O-RADS MRI для быстрой ориентации в системе и упрощения формирования заключения врачом-рентгенологом.</p></sec><sec><title>Заключение</title><p>Заключение. В результате проведенного исследования категориальная шкала риска образований яичников по данным МРТ (O-RADS MRI) показала себя надежным рабочим инструментом для связи рентгенолога и гинеколога-онколога. Однако продолжает оставаться дискуссионным вопрос о категориальной принадлежности ряда нозологических наименований, не отраженных в O-RADS MRI и требующих дальнейшего изучения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to study the informativity of the Ovarian-Adnexal Imaging-Reporting-Data System Magnetic Resonance Imaging (O-RADS MRI) in detection of ovarian cancer in the practice of a radiologist.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 271 women who underwent pelvic magnetic resonance imaging (MRI) in the period from August to November, 2021. Two radiologists with 6-year (Researcher 1) and 2-year (Researcher 2) experience retrospectively analyzed MR images, in which all ovarian masses were evaluated according to O-RADS MRI categorical risk scale from 0 to 5. The analysis of true positive, true negative, false positive and false negative rates according to MRI data compared to the reference data (histology or 1-year follow-up) was performed.</p></sec><sec><title>Results</title><p>Results. O-RADS MRI categorial risk scale had high diagnostic performance for the characterization of adnexal lesions (sensitivity 87.5% and 87.5%, specificity 97.84% and 96.75%, area under curve (AUC) 0.991 and 0.986 for Researchers 1 and 2, respectively), as well as good interreader agreement (Cohen’s kappa coefficient 0,83). Researchers’ mistakes were analyzed, as well as controversial issues of categorial affiliation of ovarian masses that caused false positive and false negative results. Visual aids for ovarian MRI in accordance with O-RADS MRI classification were made for quick orientation in the system to simplify creating the radiology report.</p></sec><sec><title>Conclusion</title><p>Conclusion. O-RADS MRI categorial risk scale proved to be a reliable working tool for the communication between radiologist and gynecologist-oncologist.
