В исследование включены 28 пациентов с метастазами КРР в печени. Больные бьли разделены на две группы: 1-я группа – 15 пациентов до химиотерапии, 2-я группа – 13 пациентов после химиотерапии со стабилизацией процесса. Во всех случаях было проведено УЗИ печени в стандартном В-режиме и с применением контрастного препарата SonoVue ® (Bracco, Italy) с регистрацией и оценкой параметров кривой интенсивность–время (КИВ). Также при УЗИ печени с контрастным усилением оценивался характер контрастирования метастазов в трех фазах – артериальной, венозной и отсроченной.</p></sec><sec><title>Результаты</title><p>Результаты. Определены три варианта накопления контраста в печеночных метастазах КРР в артериальной фазе: по периферии очагов (у 60% больных из 1-й группы, у 76,9% из 2-й группы), гомогенно по всему объему (у 26,7% из 1-й группы, у 0,08% из 2-й группы), параллельно с интактной паренхимой печени (у 13,3% из 1-й группы, у 23,02% из 2-й группы). В отсроченной фазе у 4 пациентов (14,3%) обнаружены дополнительные метастазы. При оценке параметров КИВ отмечена разница в начале стадий контрастирования у пациентов 1-й и 2-й групп: раннее контрастирование печеночных метастазов в 1-й группе (19,3 с), позднее вымывание контраста из метастазов во 2-й группе (65,9 с).</p></sec><sec><title>Заключение</title><p>Заключение. УЗИ с контрастным усилением обеспечивает лучшую визуализацию метастазов в печени по сравнению с В-режимом. Изменение сосудистой архитектоники и гемодинамики в печеночных метастазах КРР после химиотерапии отображается в изменении скорости накопления и вымывания контраста из метастазов, что позволяет использовать УЗИ с контрастным усилением при оценке эффективности лечения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to determine the possibilities of contrast-enhanced ultrasound (CEUS) in identifying and evaluating the efficiency of chemotherapy in patients with colorectal liver metastases (CLM).</p></sec><sec><title>Material and methods</title><p>Material and methods. The investigation enrolled 28 patients with CLM. The patients were divided into two groups: Group 1 – 15 pretreatment patients; Group 2 – 13 posttreatment patients with process stabilization. All the patients underwent standard B-mode ultrasound of the liver and that using the contrast agent SonoVue ® (Bracco, Italy), by recording and estimating the parameters of the intensity-time curve (CIV). Liver CEUS assesses the nature of contrasting metastases in three phases (arterial, venous, and delay ones).</p></sec><sec><title>Results</title><p>Results. The investigators identified three types of contrast agent accumulation in CLM in the arterial phase: along the periphery of the lesions (in 60% of the patients of Group 1, in 76.9% in Group 2), homogeneously over the entire volume (in 26.7% in Group 1 and in 0.08% in Group 2), in parallel with intact liver parenchyma (13.3% in Group 1 and 23.02% in Group 2). In the delay phase, more metastases were detected in 4 cases (14.3%). Estimation of CIV parameters showed a difference at the beginning of contrast enhancement stages between the patients in both groups. Group 1 exhibited the early contrasting of liver metastases (19.3 sec); Group 2 displayed the late washout of a contrast agent (65.9 sec).</p></sec><sec><title>Conclusion</title><p>Conclusion. CEUS versus B-mode ultrasound improves the imaging of liver metastases.
