Сравнить информативность применения динамической магнитно-резонансной томографии (МРТ) и методики диффузионно-взвешенной (ДВ) МРТ у пациенток с метастатическим поражением печени при раке молочной железы в процессе проводимого им химиотерапевтического лечения.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены 30 пациенток с метастатическим поражением печени при раке молочной железы в процессе проводимого им лечения. Сопоставлены результаты стандартных МРТ-исследований абдоминальной области (с внутривенным контрастированием и оценкой состояния печени в артериальную, венозную и отсроченную фазы) с данными ДВ МРТ с различными значениями b-фактора (50, 400 и 800 с/мм2).</p></sec><sec><title>Результаты</title><p>Результаты. При сравнении результатов первичных исследований, полученных до начала химиотерапии (только динамической МРТ с внутривенным контрастированием и только ДВ МРТ), полное соответствие количества выявленных очагов отмечено у 10 больных. У 10 пациенток при ДВ МРТ было выявлено большее число метастатических очагов, чем при исследовании с внутривенным контрастированием, – в целом на 44 очага больше (36 из них – размерами до 1 см). У остальных 10 (из 30) больных и при внутривенном контрастировании, и при ДВ МРТ были обнаружены множественные, неисчисляемые (более 20–40) метастатические очаги различных размеров (1–6 см). Динамическое наблюдение в процессе проводимой химиотерапии имело место у 19 из 30 наших пациенток. В 7 случаях (из этих 19) отмечена стабилизация метастатического процесса в печени, отображенная сходно по данным обеих методик. В 11 случаях (из этих 19) наблюдалось прогрессирование метастатического процесса, отображенное сходным увеличением количества и размеров метастазов по данным обеих упомянутых методик у 5 из 11 больных. У остальных 6 пациенток (из этих 11) количество вновь выявленных очагов в печени по данным ДВ МРТ оказалось больше, чем при внутривенном контрастировании. У последней пациентки (из 19) по данным обеих методик обнаружено уменьшение количества мелких метастазов в печени при сохранении их размеров (до 1 см).</p></sec><sec><title>Заключение</title><p>Заключение. Результаты применения методики ДВ МРТ свидетельствуют о значительно большей ее результативности в сравнении с данными традиционного внутривенного контрастирования, что обусловливает необходимость включения этой методики в стандартный протокол МРТ-исследований абдоминальной области у пациентов с метастатическим поражением печени.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective. To compare the informative value of using dynamic magnetic resonance imaging (MRI) and diffusion-weighted MRI (DW-MRI) in patients with breast liver metastasis during chemotherapy.</p></sec><sec><title>Material and methods</title><p>Material and methods. The investigation enrolled 30 patients with breast liver metastasis during their treatment. The results of standard intravenous contrast-enhanced abdominal MRI studies (by evaluating the liver in the arterial, venous, and delayed phases) were compared with those of DW-MRI with different B-factor values (50, 400 and 800 sec/mm2).</p></sec><sec><title>Results</title><p>Results. Comparison of the findings of initial studies prior to chemotherapy (those of only dynamic intravenous contrast-enhanced MRI and only DW-MRI) revealed a complete correspondence with the number of detected foci in 10 patients. Ten patients had a larger number of metastatic foci at DW-MRI than at intravenous contrast-enhanced MRI, in the aggregate by 44 foci more (of them 36 foci measured less than 1 cm). The remaining 10 of the 30 patients were found to have multiple, unquantifiable (more than 20–40) metastatic foci of various sizes (1–6 cm) at both intravenous contrast-enhanced MRI and DW-MRI. Nineteen of the 30 patients were followed up during their chemotherapy. Seven of the 19 patients showed a stabilized liver metastatic process that was similarly evidenced by both techniques. Eleven of the 19 patients were observed to have a progressive metastatic process that was reflected by the similar increase in the number and size of metastases in 5 of the 11 patients, as shown by both of the above techniques. In the remaining 6 of the 11 patients, the number of newly detected liver tumors proved to be larger at DW-MRI than at intravenous contrast-enhanced MRI. Both techniques showed that the last patient of the 19 cases had a decrease in the number of small liver metastases that maintained their sizes (less than 1 cm).</p></sec><sec><title>Conclusion</title><p>Conclusion.
