В 2015–2016 гг. в МОНИКИ им. М.Ф. Владимирского в предоперационном периоде обследованы 65 пациентов при помощи КТА и УЗИ. Оценка степени стеноза проводилась по критериям NASCET (North American Symptomatic Carotid Endarterectomy Trial) и ECST (European Carotid Surgery Trial). Для характеристики данных КТА применялся специализированный протокол.</p></sec><sec><title>Результаты</title><p>Результаты. Атипичное отхождение брахиоцефальных артерий выявлено в 26% случаев, в 51% случаев – сочетанное поражение внутренних и общих сонных артерий, подключичных артерий. В 29% случаев отмечены атеросклеротические бляшки (АСБ) на уровне верхних отделов общих сонных артерий, что повлияло на проведение оперативного вмешательства. У пациентов, участвовавших в исследовании, был выявлен стеноз внутренней сонной артерии (ВСА) более 50% по NASCET. Подробно оценивалось состояние ВСА выше уровня луковицы. У 62% пациентов с критическим стенозом ВСА было выявлено уменьшение калибра артерии по сравнению с противоположной стороной. В 3 случаях от пораженной ВСА отходила восходящая глоточная артерия, что повлияло на тактику хирургического вмешательства. Все пациенты в исследуемой группе были прооперированы. Получено соответствие степени стеноза по данным КТА и интраоперационным результатам. Данные КТА о протяженности поражения ВСА оказались менее информативными: в 40% случаев интраоперационная протяженность АСБ превышала таковую при КТА.</p></sec><sec><title>Заключение</title><p>Заключение. КТА является информативным исследованием, позволяющим выполнять детальную оценку окклюзирующего поражения сонных артерий для планирования оператив-ного вмешательства. </p></sec></abstract><trans-abstract xml:lang="en"><p>Objective – to optimize dedicated assessment of carotid arteries in case of occlusive disease with computed angiography (CTA).</p><sec><title>Material and methods</title><p>Material and methods. During 2015–2016 years 65 patients underwent ultrasound examination and computed tomography examined before carotid surgery.</p></sec><sec><title>Results</title><p>Results. Aortic arc branching variants were observed in 26% cases. In 51% combined steno-occlusive process in common, internal carotid arteries and subclavian arteries was confirmed. In 29% of cases atherosclerotic plagues were detected in upper parts of common carotid arteries and such data changed surgical strategy. Patients included in study has internal carotid artery (ICA) stenosis more than 50% NASCET. Detailed assessment was used for cervical ICA distal to stenosis. In 62% of patients with carotid subocclusion narrowing in upper segments were detected. In 3 cases ascending pharyngeal artery arose from affected carotid artery and these findings change surgical approach. All patients underwent carotid surgery. Degree of ICA stenosis detected with CTA was conformed. CTA data for carotid plague extension were less informative – intraoperative plague length exceeded CTA results.</p></sec><sec><title>Conclusion</title><p>Conclusion.
