Несмотря на значительный прогресс в диагностике и лечении РМЖ, показатели смертности от этого заболевания остаются высокими, что может быть связано с несовершенством предоперационного обследования при планировании лечения и, как следствие, возникновением локального рецидива заболевания и развитием отдаленных метастазов. Несмотря на то что маммография является «золотым стандартом» в диагностике рака молочной железы на ранней стадии заболевания, существующие ограничения метода, такие как плотный рентгенологический фон молочной железы, требуют поиска дополнительных методов исследования для проведения не только скрининга, но и предоперационного обследования пациенток с РМЖ. При планировании лечения требуется высокоточная предоперационная диагностика, позволяющая оценить такие важные параметры, как распространенность, мультифокальность и мультицентричность заболевания. Наиболее подходящим методом для этих целей может быть магнитно-резонансная томография (МРТ) молочных желез с внутривенным контрастированием. Целью настоящего обзора литературы был анализ результатов проведенных исследований, касающихся необходимости рутинного применения МРТ молочных желез и наличия преимуществ этого метода в предоперационном обследовании пациенток с РМЖ. Анализ литературы показал, что в настоящее время нет единого мнения относительно роли и места МРТ молочных желез в предоперационном обследовании пациенток, страдающих раком молочной железы. Результаты многочисленных исследований демонстрируют абсолютно полярные выводы, что, в свою очередь, требует проведения новых исследований. </p></abstract><trans-abstract xml:lang="en"><p>Breast cancer represents a global healthcare problem. In spite of all advances in treatment regimens for breast cancer, mortality from the disease stays high in countries where population based screening has not yet been introduced. On the other hand, there is a significant decrease in mortality from breast cancer in those countries where treatment is applied in an early phase of the disease, as a result of regular, population-based screening with mammography. Although mammography is the gold standard in diagnosing breast cancer in its early stage, it has limitations in detecting breast cancer sufficiently early in the dense portion of the breast. This calls for using the multimodality approach also for screening. In addition, the high frequency of multifocal and diffuse breast cancers calls for the need to describe the true extent of the disease preoperatively. The best modality to accomplish this goal is the use of magnetic resonance imaging of the breast (MRI).
