Материал и методы. В покое и на фоне физической нагрузки 18 пациентам с идиопатической легочной гипертензией выполнена перфузионная ОЭКТ миокарда с 99mТс-МИБИ, с КТ-коррекцией поглощения излучения (AC), с ЭКГ-синхронизацией. Обработку томосцинтиграмм выполняли в ручном режиме, с раздельной реориентацией и полуавтоматическим оконтуриванием обоих желудочков. Расчет коэффициента относительной интенсивности накопления МИБИ в правом и левом желудочках (RV/LV) проводился по собственному методу, в сравнении с другими описанными способами. Результаты. У пациентов с ЛГ, по данным перфузионной ОЭКТ миокарда, отмечаются патологическая визуализация правого желудочка (ПЖ), его дилатация и снижение сократимости, парадоксальное движение и характерные дефекты перфузии межжелудочковой перегородки. Оконтуривание ПЖ в полуавтоматическом режиме оказалось возможным у 100% пациентов. Средние значения коэффициента RV/LV у пациентов с ЛГ и в норме составили 0,6097 ± 0,0090 и 0,2750 ± 0,0355 соответственно (p&lt;0,001). Заключение. Новый способ расчета количественных пара- метров нарушений перфузии позволяет выявить особенности перфузии миокарда правого и левого желудочков у пациентов с легочной гипертензией, с потенциальной возможностью оценки эффективности терапии у этих пациентов. Новый способ расчета количественных параметров нарушений перфузии позволяет выявить особенности перфузии миокарда правого и левого желудочков у пациентов с легочной гипертензией, с потенциальной возможностью оценки эффективности терапии у этих пациентов. </p></abstract><trans-abstract xml:lang="en"><p>Objective: to develop and implement a new processing approach of myocardial perfusion images obtained by single-photon emission computed tomography (SPECT) in patients with pulmonary hypertension (PH). Material and methods. 18 patients with idiopathic pulmonary hypertension underwent gated rest-stress perfusion SPECT with 99mTc-MIBI with CT-attenuation correction. Images were processed manually, with separate reorientation and semi-automatic contouring of both ventricles. MIBI intensity ratio in the right and left ventricles (RV/LV) was calculated using novel volumetric method, in comparison with other described approaches. Results. In patients with PH myocardial perfusion SPECT revealed abnormal RV visualization, dilation and reduced contractility, paradoxical motion and typical perfusion defects of interventricular septum. RV contouring in semi-automatic mode was possible in 100% of cases. The average values of RV/LV ratio in PH patients and normal group was 0.6097 ± 0.0090 and 0.2750 ± 0.0355, respectively (p&lt;0.001). Conclusion.
